Varsity Jacket Order Form

Varsity Jackets are presented by the Millis Booster Club to all Student Athletes who have earned a minimum
of two varsity letters or will be participating in a Varsity sport during their senior year.

Juniors wishing to have their jackets for their senior year, must order them at the beginning of the
Spring Sports Season in order to receive them at the end of year Awards Night.

Please complete this form and return it to the Athletic Directors office within one week
of the close of the season of which you became eligible.
You will receive your jacket at the end of the next seasons Awards Ceremony.
