The user fee is now $220 per athlete per season with a 4 season cap per family per year. (not including any additional sport specific fees)

A very important reminder for middle and high school students intending to participate in a fall sport: Registrations are due NO LATER THAN FRIDAY AUGUST 20th!!
***Tryouts/practices begin Monday 8/23 for football; Thursday 8/26 for all other sports. No athlete will be permitted to practice/tryout without a completed registration & CURRENT PHYSICAL!!
Turning in packets:
Please fill out the packets so that the information is current when the fall season starts.
Completed registrations, physicals, and checks may be:
1. Dropped off at Coach Grant’s office or the Athletic Training Room
2. Dropped off Monday 8/16 through Wednesday 8/18 from 5-6:30pm in front of the gym.
3. Mailed in to: Jamie Farias, Athletic Trainer; 245 Plain Street, Millis, MA 02054
4. Faxed to Attention: Jamie Farias at (508)376-7020.
I will be sending lists to coaches of who is eligible and who is not. It is the athlete’s responsibility to make sure they have all required items turned in prior to the first tryout/practice.
**Registration packets will only need to be turned in once per school year!** However, a new physical is always required if the one on file expires!!
A COMPLETED REGISTRATION INCLUDES:
- The 4-page registration packet filled out and signed by parent/guardian and athlete. This includes a registration/emergency info/permission form, liability release, and medical history.
Please be sure to write in which sports the athlete is planning to participate in for the entire school year 2010-2011!
1. A check for $220 made payable to “Millis Athletics.” Please write the athlete’s name & sport in the memo line!
2. A copy of the athlete's current physical exam. A current exam for this fall must have been performed ON OR AFTER JULY 23RD, 2009. If you are not sure if the current physical is on file with the ATHLETICS Department, please turn in a copy to Athletics with the registration forms, or contact me. I will pass all physicals along to the school nurse.
**Please also make sure that if the athlete is under a physician’s care for an injury or other problem that occurs during the summer and restricts their activity; that at the start of the season they obtain written clearance from that physician to return to sports.**
A note on forms:
The registration packet will be kept on file in the athletic training office. The medical history form will help me to better care for your child by providing more thorough background information on your child’s illness, injury, and overall health history. Most, if not all, of this information is not provided on your child's physical exam from their doctor. This information helps in all aspects of care, from injury and illness prevention and evaluation, to potential physician referral, to treatment and rehabilitation, and a safe and healthy return to sports. Physical Exam forms will be kept in the nurse’s office.
A copy of the Emergency Information Card will be kept in an envelope in each teams’ first aid kit. These kits travel with your child’s team to practices and home and away contests. In the event of an emergency, the cards will be used to help the medical staff and coaches contact parents, and provide the necessary information about your child to emergency medical services (EMS).
Varsity Letter Jacket Order Form
Varsity Jackets are presented to Student Athletes by the Millis Boosters club once they have earned two varsity letters. The varsity letters do not have to be in the same sport. The jackets are donated by the Boosters, therefore, they are free to eligible athletes. Print the form and return to athletic director for approval within one week of the close of the regular season when you became eligible.
